Will be required at check-in. If you arrive at camp without these forms, you will be sent home. Please double and triple check that you have your forms before leaving for camp. You will not receive a refund due to pre-planning and purchases based on the registrations.

- IH/COR must submit documentation of their knowledge and approval your unit will be attending Summer Camp. DUE
BSA Annual Health and Medical Form PARTS A,B,C - PDF
- MANDATORY and must be brought with each camper and participant. Cannot accept these forms electronically. Make sure you have the latest uptodate version.
